Practice the 21st Century Approach Age management medicine is just that—scientifically sound, evidence-based medicine. It is not part of the anti-aging movement, nor does it claim to increase longevity or affect aging at cellular levels. Equally important, it is not founded on unsubstantiated claims or pseudo-science. This medical specialty regards aging as a natural process (not a disease) that can be managed with innovative programs. Trained age-management physicians incorporate well-known and accepted markers of disease risk into proactive patient management, making it possible to prevent premature disability and death while helping patients regain and maintain optimal health and vigor. Certified age-management physicians use comprehensive evaluation to guide patients on a synergistic, integrated approach using low-glycemic nutrition, exercise, nutraceuticals, lifestyle adjustments and hormone optimization, when clinically indicated. When clinically indicated, the physician uses hormone optimization for the endocrinologically “normal” by identifying hormone levels, which yield superior health outcomes. For most hormones, that is the upper 25% - 30% of the normal range for the patient’s age. The exceptions are insulin, cortisol and hGH. Reducing insulin and cortisol levels to the lower end of the normal range for the patient’s age is key—whereas hGH optimization is considered only when a comprehensive evaluation reveals an adult growth hormone deficiency (AGHD). Backed by extensive training, age-management physicians can accurately evaluate a patient’s health status in terms of age-management goals and initiate an appropriate, individualized hormonal optimization program. The physician evaluates hormones that decline with age, the timing and degree of that decline as well as the related physiologic consequences—knowing hormone optimization’s risks and benefits, regarding sex hormones, thyroid hormone, melatonin and growth hormone (for diagnosed AGHD). Controlling levels and achieving balance with hormones contributing to the aging process are key. Patients are monitored to determine the treatment’s efficacy with periodic diagnostic testing to determine progress objectively and needed adjustments in hormone dose and/or related therapy, addressing any side effects to keep patients as disease-free as possible.
