![]() PRIVACY AND CONFIDENTIALITY POLICY CME Participants: When you register on our website for a CME activity, we collect certain personally identifiable information from you such as your name, email address and mailing address. In addition, we collect aggregated non-personally-identifiable information that we use in several ways. Cenegenics Education and Research Foundation (CERF) courses are accredited by the Foundation for Care Management (FCM). As such, CERF is required to submit aggregated data about CME participants to FCM. We also maintain personally identifiable information about you and credits issued to you for the purpose of maintaining records that you can request from us and/or FCM for up to six (6) years. CERF and FCM support staff will have internal access to files containing personally identifiable information, including evaluation forms and aggregated CME participant information. These files can be accessed in order to respond to questions and/or comments. Aggregated data may also be used to assess educational needs as well as to plan marketing activities. Individual identifiable information is not shared with outside parties, nor does CERF or FCM sell or provide email or mailing lists to third parties. General Privacy Policies: CERF and FCM have implemented security policies and procedures to protect all personal data from unauthorized access, improper use and accidental loss or destruction. We also require all of our employees and others who have access to or who are associated with the processing of your data to agree to maintain the integrity and confidentiality of your information.
